Return Material Authorization (RMA)

Please fill in the following Return Material Authorization (RMA) form if your equipment requires service.
After submitting the form, a confirmation email will be sent to the email address you provided.
DO NOT send the product until a Fiplex representative contacts you with an RMA number and return address where you can send the equipment.
You must enclose the RMA number with the product when returned as it will be used as a reference when inquiring about the repair status. Please note that returned products without an RMA number will not be processed.
